The Seneca County Health Department and Common Ground Health are gathering “health stories” from Seneca County residents in an attempt to learn more about our residents’ health-related needs. The results of this survey weigh heavily on the priorities that Seneca County Health Department and our healthcare partners will set towards addressing health concerns for the next few years. Please take a few moments to tell us your “health story” through the survey (linked below). Please share the link with as many family, friends, and colleagues as you wish. The more surveys we receive, the more accurate we can be towards setting goals in improving the health and wellbeing for Seneca County residents. The survey will only take about 10-15 minutes of your time. If you wish to take the survey on paper, you can find a version available for printing attached. Please return completed paper surveys by mailing them to the attention of Kerry VanAuken, Seneca County Health Department 2465 Bonadent Drive Suite 3, Waterloo, NY 13165 or by calling (315)-539-1920 to schedule a pick up.
You can complete the survey anonymously, or enter your name and email for a chance to win a $500 gift card to a local supermarket or gas card. **
